New Patients
How To Register
From the 31st October 2024 – patients can now register at our surgery using the new NHS “Register with a GP Surgery” service.
Please note that Broomhill & Lodge Moor Surgeries are part of the same GP practice so by registering with either surgery online, you will be able to access services at both surgeries.
If you are unable to complete the online registration you can still complete a PRF1 Registration form above and bring it to reception along with proof of ID to reception to confirm your address.
If you have misplaced your medical card you will be asked to fill out a Registration form
We also request that you complete a New Patient Questionnaire which is available to download by following the links above, or collected from the surgery reception

New patients can register with us online using the NHS App or by visiting the NHS Website
