If you would like to be assessed for ADHD via the Right to Choose (RTC) pathway please let the practice know either by calling and informing a receptionist, notifying us via the practice website or sending a message using the “contact us” section of the website.
You do not need to book an appointment.
We will send you a referral form to complete and will send it to your chosen provider when you have returned it, however if you are prescribed a medication you need to be aware that the practice will not take over prescribing under what is called a Shared Care agreement. ADHD medications are for specialists to prescribe that some RTC ADHD providers can supply as a cost.
